The Submit Community Report form is a quicker, lighter-weight way to let Serve Source's team know about a disaster response or impacted area — including on behalf of an organization you don't have login access to. It exists so that no need or effort falls through the cracks just because the person aware of it doesn't have that organization's account — a church member who's seen a need, or a volunteer who knows what a small local group is doing, can flag it without needing to be that organization's own staff. It's reviewed by Serve Source staff before anything is published.
- Log in (an account is required, but you don't need to belong to the organization you're reporting on).
- Go to Submit Community Report.
- Fill in the form:
- Organization Name and, if you have it, the organization's official website
- Organization Type
- Whether you're acting on behalf of that organization (if so, a contact phone number is required)
- Your email
- The disaster this relates to
- Response Scope (Disaster Response or Rebuild — see How to Submit a Long-Term Rebuild Project for the difference)
- Services being offered
- Start date, and end date if known
- A description of the response
- Location details (name, address/area, and GPS coordinates if you have them)
- Who should see this resource
- Submit the report.
Serve Source's team reviews every submission before it's published, so there may be a short delay before it appears on the site.
